The UBC Health Wellbeing and Benefits, Occupational and Preventive Health, and Pharmacists Clinic teams are working together to provide UBC staff and faculty relevant and timely vaccine-related information and access to local vaccination services.
We are conducting a short survey on your opinions about vaccinations. Your responses will help us understand your areas of concern and ways we can better meet the vaccination needs of UBC staff and faculty. The study’s principal investigator is Adrian Ziemczonek, Clinical Pharmacist and Lecturer at the UBC Pharmacists Clinic.
The survey consists of 14 questions, including topics related to COVID-19 outbreak, and should take less than five minutes to complete. You can skip any questions you do not want to answer or are not relevant to you. Your responses are anonymous and participation is voluntary.
The survey will close July 12, 2020.
