Provincial drug integration form

Some prescription drug types require special authority approval from BC Pharmacare in order to receive coverage.

If you are prescribed a drug that can be covered under the provincial special authority drug program, Sun Life may require you to complete and send them the Provincial Drug Integration Form to be eligible for reimbursement under the extended health plan. This ensures reimbursement for prescribed drugs are properly coordinated with the provincial drug programs and your extended health plan.

A section of the form is required to be completed by your prescribing physician. The form can be completed and signed electronically.

Example: Nagamo has a respiratory condition and has been working with their physician to manage their symptoms. After trying other medications without much success, Nagamo’s physician suggests a medication which requires special authority approval from BC Pharmacare in order to be covered. Working with their physician on completing the provincial special authority request form, Nagamo receives approval from BC Pharmacare, and can now go to the pharmacy to have their prescription filled. The pharmacist notifies Nagamo that their extended health plan requires the Sun Life provincial drug integration form to be completed in order to receive reimbursement for their prescription. Nagamo completes the provincial integration form from Sun Life with their physician and uploads both the form and their proof of provincial approval through their Sun Life mobile app and submits it for processing.  

Sun Life reviews Nagamo’s forms and approves their prescription for reimbursement under their extended health benefit plan. As Nagamo is a CUPE 2950 employee, they can also submit any cost of having their physician complete both forms through their Health Spending Account for reimbursement. 

Submitting the form to Sun Life

Once you and your prescribing physician have completed the form, you can submit the form to Sun Life through your online Sun Life account (www.mysunlife.ca), the Sun Life mobile app or by mail to the nearest address provided at the bottom of the second page of the form.  Please ensure you include the province's response to your provincial specialty drug program application, if applicable.

To submit the completed form through your online Sun Life account,

  1. Select ‘Drug lookup’
  2. Then ‘Submit a drug form’.
  3. When selecting what this form is for, please select ‘Provincial specialty drug response’
  4. Then upload your file.

To submit the completed form through your Sun Life mobile app,

  1. Select ‘Documents’ from the navigation bar,
  2. Then ‘Benefits’ and ‘Medical or dental plans’.
  3. For document type, select ‘Drug forms’ and select ‘Drug Exception Application’ as the document type.
  4. Then upload the photos of your form.

If your physician charges for completion of the form and you are part of an employee group with a Health Spending Account (HSA), you will be able to submit the charge to Sun Life for reimbursement.
